**Position Overview:**

**PRIMARY PURPOSE:**

Under the supervision of the Director of Adult Services, Van Ness

Campus, the Dialysis Navigator RN functions within CPMC to assure patients with

diagnoses of Chronic Kidney Disease and End Stage Renal Disease are provided

with care coordination and patient education services. The Dialysis Navigator RN will utilize the

nursing process to develop interventions to assess, intervene, educate and,

actively plan care with the patient(s); the RN will also collaborate with other

care providers (both in the hospital and in the community) to ensure quality

outcomes for their patients.The coordinator

will also function as a patient advocate for patients with renal disease.

**Qualifications:**

**POSITION REQUIREMENTS** :

**Education:**

A Bachelors degree in Nursing or a related field is required.

Masters preparation in aspects of cardiac disease is desirable.

**Experience:**

Registered Nurse with an active license in the State of California.

A Bachelors degree in Nursing or a related field is required.

Clinical nursing experience in dialysis and/or counseling is strongly preferred.

Experience with public health, data and record keeping, program development, patient education and/or project management desired.

**Knowledge:**

Excellent interpersonal, public speaking and teaching skills.

Ability to comprehensively assess, plan, intervene and evaluate the physical, emotional, psychological and educational needs of patients

Knowledge about current dialysis treatment, family dynamics and associated quality of life issues is preferred.

Commitment to continual improvement in clinical skills and knowledge base.

Microsoft Office software experience required.

Secondary language skills desirable.
